1) Go to hmsa.com/hra and click on the “health risk assessment questionnaire” link.

2)      Logging in:

If you are a returning member, click the Continue button and follow the screens to access your account.

If this is the first time you are taking the HRA, scroll through the privacy statement and agree to the Terms and Conditions, then click the Next button.

3)      Complete the Eligibility Confirmation section and then click Next.

4)      You will see a window asking for a promotion code. IMPORTANT - You must enter HICOUNTY in the promo code field; then click on Next or Continue.

5)      Create your userid, password and security question, then click Next.

6)      Click the Start Health Risk Assessment button to start your questionnaire.
