Kaiser Permanente - Taking Your HRA |

To take Kaiser Permanente's Health Risk Assessment, (also known as a Total Health Assessment), you'll need to first register on kp.org's My health manager, if you don't already have a user ID and password. You'll need your Kaiser Permanente health record number to sign up.
In addition to accessing the Total Health Assessment, My health manager will enable you to email your doctor, check lab results online, renew prescriptions, access parts of your child's medical record, and so much more.
Step 1: Set up your kp.org My Health Manager user ID and password*:
- Go to kp.org/register
- Create your own user ID.
- You’ll then be asked four to five authentication questions that only you should know the answers.
- Once you answer the questions correctly, you’ll be assigned a temporary password. (If you fail to answer the questions correctly, you’ll receive a temporary password in the mail in three to seven days.)
- Once you receive your temporary password, go to kp.org/password as soon as possible and enter your user ID and temporary password.
- Change your temporary password to one of your own and you’ll be ready to complete the HealthMedia Succeed Total Health Assessment.
- If you have previously registered on kp.org but forgot your password, you will be able to get a new password by requesting online or by calling our kp.org Helpline at (* kp.org help line: 1-800-556-7677).
Step 2: Once you are registered on kp.org, just follow these simple steps:
- Log onto kp.org/healthylifestyles
- Click on “Get a personalized total health assessment.”
- Enter your user name and password and click “sign on”. If you don’t already have password, follow the instructions above.
- Click “Continue on to healthy lifestyles program.”
- Click “Health Media® Succeed™” on the left side of the page.
- Complete your questionnaire & click to share results with your Kaiser Permanente physician.
- At the end of the questionnaire you’ll receive a personalized report.
